Consent to personal data processing

I hereby agree to processing by the Data Controller - the Mayor of the City of Poznań with the seat at Plac Kolegiacki 17, 61-841 Poznań - of my personal data in the form of my e-mail address in order to use the newsletter service of the Urban Multimedia Information Service (Polish: Miejski Informator Multimedialny; www.poznan.pl)
I hereby acknowledge that:
1. A Data Protection Officer was appointed and you can contact them by e-mail: iod@um.poznan.pl or in writing at: plac Kolegiacki 17, 61-841 Poznań
2. Data obtained for the purposes of rendering the service are stored until they are removed by the user or by the data controller at the request of the user and they are not archived.
3. The consent to personal data processing may be rescinded at any time without having any impact on the legality of processing that has been carried out pursuant to a consent to data processing before it was rescinded
4. Data subjects have the right to:
a) access their personal data
b) request rectification of their data if the data are incorrect
c) request erasure of their data, if:
- the data are not necessary for the purpose they were collected
- after the rescission of the consent to data processing
- data are processed contrary to the law
d) request restriction of processing, if:
- the data subjects challenge the accuracy of the data
- the processing is contrary to the law, and the data subjects object to erasure of data
- Data controller no longer needs personal data for the purposes of processing, but the data are necessary for the data subjects to establish, enforce, or defend claims
5. I have the right to file a complaint with the supervisory authority, i.e. the President of the Personal Data Protection Office
6. Providing my e-mail address is voluntary but necessary to render the newsletter service of the Urban Multimedia Information Service.
7. Personal data will not be processed in a way that is based solely on automated processing.
8.
